A physical check-up certificate is necessary for internships, graduate school scholarship applications, or applying for a job, etc.

Please note: This certificate is available only if you complete the two medical check-up sections.

Automatic issue of certificates

The period available for automatic issue of certificates is announced on the portal board.
The certificate machines are located in “Yume-kukan” and at the east exit in building No. 19.

Please note

Be careful if your ID number changes during the April promotion.
If your ID number changes, the automatic issue is a certificate is not possible after April.
Please obtain your certificate beforehand if you need it soon after the start of the new academic year.

In case you need a certificate in a specific application format

Bring the example of the specific format required to the center reception counter, along with your certificate printed by the machine. The certificate items will be restricted to the NITech version. This procedure takes approximately three days.

In case you need a certificate without completing the medical check-up at this university

We can introduce a clinical hospital for your health check-up. After the medical check-up, ask the clinic to provide you with a certificate. Of course, you will need to pay the entire cost.

For non-degree research students

The automatic issue of a certificate is not available. Apply for the certificate at the center reception counter, and you will receive a certificate in approximately three days time. This certificate is available only if you complete the two sections of the medical check-up.
